Have you submitted your health forms yet? At Cedar Crest College we focus on the health and wellness of our students. All new students must complete the mandatory health forms prior to starting classes in the fall and spring. Please access Medicat through FalconLink to complete your health forms. New student health forms are due: August 1st or two weeks after enrolling.

Please note that most information can be completed right online. Some forms need to be completed and uploaded to Medicat. Please make a copy of your health forms for your personal records.
